New Patients

If you wish to be seen regularly, you will have to join the practice list. Once you are registered with the practice you are free to see any doctor you choose. Please note it is always to best to try and see the same doctor throughout a period of treatment to ensure continuity of care.

If you are registering your baby then please bring along the form given to your by the Registrar of Births.

If you are moving from another doctor please bring your medical card. This is a yellow/buff card issued by the Health Board where you last lived.  Please sign this card and hand it to one of the receptionists. Don’t worry if you can’t find your medical card, as we can give you a form as an alternative.

Please bring with you 2 forms of identification – one must be photographic and the other with proof of your address.

Patients can complete the permanent GP registration form online and then email it to the practice at nhsh.gp55841-reception@nhs.scot.  A link to the permanent GP registration form is below:

When you have completed the paperwork we will offer you an appointment with either a Doctor, Nurse or Healthcare Assistant.

Leaving Our List

You can leave the doctor’s list at any time. This may be because you are moving away. You do not have to tell us you are leaving, but it would be helpful to us to know.

In very rare circumstances you may be removed from the list. This would only occur when we feel that the trust that must be present between you and the doctors no longer exists. Any abuse of, or violence towards the staff or doctors will lead to immediate removal.
