Community Link Worker Team – Available in the Practice

One of the services that you have available within our Practice is Community Link Workers.  Please click on the link below to watch a short video about the service they offer. If you feel this service could be useful for you, please call the practice to make a telephone appointment with one of our GPs to discuss a referral.

“All information patients share with the Community Link Worker Team is treated confidentially. However, if there is a risk of harm to an individual, or of harm to others, then the CLW team have a duty to share that information with an appropriate referral organisation, such as the GP Practice, Emergency Services, or Mental Health services.”
